This is the only form necessary for submitting a common assessment to become a part of the Local Assessment System. Assessments should be submitted digitally through email if possible. Also, please read the document, MSAD #11 Local Assessment System Explained. This is a redefinition of our local assessment system. This document outlines the purpose and procedures involved in our local assessment system. These documents were updated on 1/28/08.
