Please turn it in to the office as soon as it is completed.

Please remember you must have a physical on file with us to try out.  The physical has to have been done within the last three years.  Please call me at 582-1326 if you are unsure about the physical as there are no exceptions. Your doctors office can fax it to us at 582-6823 but please follow up on the fax as it does not always make it to us.
