Please complete this form (https://bit.ly/3VlI34i) for each school aged child in your household. The results will determine how much funding MSAD 11 will get from the State of Maine. If we don't have accurate information or have a low return rate, we may get less from the State and that will impact how much is paid locally for schools in taxes.
This form is not for school lunch purposes, only to determine economic disadvantaged status for State funding. Your information will be kept confidential.
Please submit a form for each of your students. If possible, complete the survey by the end of the day on Friday, October 2nd.
